Who we are
"We" and "us" means Revitalise Clinic Ltd, trading as Revitalise Clinic, company number 11314843, at 4 Hopwood Lane, Halifax, West Yorkshire, HX1 5HW. You can reach us on 01422 525503 or at info@revitalise-clinic.co.uk.
Booking
You book online through Timely, our booking system, or by calling us. When you book online, you are using Timely's booking pages, and Timely's own terms also apply to that part. Your booking is confirmed when you receive a confirmation from us or from Timely.
Please give us accurate contact and health details when you book and at your consultation. We rely on them to look after you safely.
Treatments are for adults aged 18 and over only. We do not treat anyone under 18, even with a parent or guardian.
Consultation and suitability
Every treatment starts with a consultation. The clinician who sees you asks about your health and medicines and decides whether the treatment is suitable for you. Some treatments involve prescription-only medicines, and a qualified prescriber must agree that they are right for you before they can be given.
We may decline or postpone a treatment if we think it is not suitable or not safe for you, or if we need more information first, such as blood test results. If that happens before any treatment is given, you will not be charged for the treatment.
Prices and payment
Our current prices are on our prices page and the treatment pages. The price for your appointment is shown when you book. If anything changes after your consultation, we will explain why and agree it with you before going ahead.
Most treatments need a £25 deposit when you book, and the rest is paid at your appointment. Injections are paid in full when you book. We do not sell gift vouchers.
Cancelling or moving an appointment
If you cannot make your appointment, please let us know as early as you can, by calling 01422 525503 or through the link in your booking confirmation, so the time can be offered to someone else.
If you cancel or move your appointment with at least 24 hours' notice, you get a full refund of anything you have paid. If you cancel with less than 24 hours' notice, or do not attend, your deposit or payment may not be refunded.
If we need to cancel or move your appointment, we will tell you as soon as we can and offer you another time.
Arriving late
If you arrive late, we may need to shorten your appointment or move it to another time, so that the next patient is not kept waiting.
Refunds
Deposits and payments are refunded in full when you cancel with at least 24 hours' notice, or when we decline or postpone a treatment before it is given.
Nothing in these terms affects your legal rights as a consumer.
What to expect from treatment
Everyone is different, so we cannot promise any particular result from a treatment. Your clinician will explain what the appointment involves, and any risks, before you decide whether to go ahead.
Aftercare
You will be given aftercare information at your appointment. Please follow it, and contact us if you have any questions or concerns afterwards.
Photographs
We only take photographs as part of your treatment record with your consent. We never publish a photograph of you without your separate written consent, which you can withdraw at any time.
Complaints
If you are unhappy with any part of your care, please tell us. Speak to us at the clinic, call 01422 525503 or email info@revitalise-clinic.co.uk. We will look into it, and reply to you.
If you are not satisfied with our reply and your concern is about a pharmacist, you can raise it with the General Pharmaceutical Council, the regulator for pharmacists, at pharmacyregulation.org.
